Examples of 10 Minute Management: Referrals

• Sometimes the issue is that you can't work with the person.
  Don't waste time - recognise your limitations.
  
  Can I work with this person: too obnoxious, too demanding, too smelly:
  the referral pathway may help you to end it.
  The more tolerant you are,
  the more patients you will accumulate that you need to be tolerant of.
  
  

• I want to see a cardiologist at the hospital.
  Option one: send them off to hospital immediately,
  no questions asked with a suitable comment on their referral –
  the patient has requested your assessment.
  
  Option two: you need this test done first – then you need this test done – then you need this done.
  Option three: Manage them when they bounce
  Option four: do not argue with them: not an option.

 

• Question: Are you suicidal? If they are, send them to hospital immediately.
  All you need to do is to refer them – so you don’t really need to take much comprehensive history.
  The hospital will do it for you- enough to get you out of trouble/ so use them as a resource.
  
  You can work with them once the hospital assesses them and sends them home.
  But if you attempt to assess them – and get this wrong –
  you are responsible for them harming themselves.
  Even if you do a long consultation and a mental health care plan –
  if they hurt themselves you are responsible. Don’t waste your time – refer.
  • Epilepsy
  • Heart Attack
  • Chest Pain
  • Abdominal Pain
  • Stroke
  • Suicidal or Deliberate Self Harm
